As a resident of Missouri, you should take the following steps to ensure your decision to become a donor is carried out:

• Sign up online at www.missouriorgandonor.com or by downloading the available form at http://www.dhss.mo.gov/OrganDonor/Reg.pdf,

or
• Sign up for the Missouri Organ Donor Registry through Department of Revenue when you apply for or renew your driver's license or non-driver's identification card.
